Reimbursement and Compensation Policy

Fair and adequate reimbursement for infectious diseases physicians is a key IDSA policy priority. In the public policy arena, the Society promotes the value of ID physicians and supports access to and appropriate reimbursement for infectious diseases services. IDSA also supports funding and policies that foster ID training. The Society also advocates for access to appropriate treatments for patients with serious infections. IDSA’s positions on specific reimbursement and compensation issues that impact the practice of infectious diseases and advocacy alerts to key policy priorities are provided below. 

 

 

 

Key policy priorities

  • In mid July of 2025, CMS released the CY 2026 Medicare Physician Fee Schedule Proposed Rule. IDSA has drafted a response in opposition to the Practice Expensive proposals which will result in a 9% cut to all facility-based ID physicians. In the letter, IDSA urges CMS to work with ID physicians to better capture specialty impact data when drafting any future proposals. 

  • In July 2025, CMS released the CY 2026 Hospital Outpatient Prospective Payment System (OPPS) proposed rule. IDSA's comment letter addresses key issues including site-neutral payment for drug administration, vaccine access, 340B payment adjustments, and the importance of maintaining strong infection prevention measures for healthcare workers.

  • IDSA President Steven Schmitt, MD, FIDSA, submitted a letter to the Senate Finance Committee leadership in response to their white paper on Medicare physician payment. In the letter, IDSA urged Congress to include ID physician incentives in upcoming payment model changes.

  • IDSA compiled a summary of the CY 2026 Medicare Physician Fee Schedule Final Rule including information on the practice expense reimbursement cuts, telehealth updates and quality measures.

  • IDSA submits comments to the Medicare deregulation RFI released by CMS.

  • IDSA submitted comments to Administrator Oz regarding the fiscal year (FY) 2026 Hospital Inpatient Prospective Payment System (IPPS) Proposed Rule.

  • IDSA and other stakeholders released a letter to congressional leadership regarding proposed changes to Medicaid that would impact access to care.

  • 4/11/2025 IDSA submitted comments ahead of the ACIP meeting in April 2025 highlighting key ID topics.

  • The addition of HCPCS code G0545, which recognizes the complexity of ID care, has further strengthened efforts to expand access to ID services.

  • Medicare and Medicaid Programs; CY 2025 Payment Policies Under the Physician Fee Schedule and Other Changes to Part B Payment and Coverage Policies; Medicare Shared Savings Program Requirements; Medicare Prescription Drug Inflation Rebate Program; and Medicare Overpayments

  • IDSA sends key ID/HIV priorities to new CMS Administrator Dr. Oz.
