All volunteer members are required to disclose any potential conflict of interest in order to participate in Society activities. The Conflict of Interest (COI) Disclosure Form must be completed by each volunteer member annually. Please remember to consider any activities and financial relationships/investments that may apply.
Below you will find instructions on how to complete and submit the form:
- Open and download the COI Form (Word document)
- Read, complete, sign, and date the form
- Save the completed form as LastName.FirstName.COIForm.Year (Example: Doe.John.COIform.2019.docx)
